The survey takes about 15 minutes to complete and is divided into 3 sections:

..... Section A asks you to describe your (the Parkinson's patient) access to health services from a list 14 types of services potentially needed by patients.

..... Section B asks for background information on the patient.

..... Section C asks for your contact information (patient or care-giver).
